The Holistic Assessment & Rapid Investigation (HARI) Service provides holistic assessment, diagnostics, rehabilitation, and care planning at the Nelson Health Centre, with visits to patients' homes as appropriate. HARI brings together a range of Adult Services to support frail adults to optimise their independence, health, and wellbeing, and prevent admission into hospital or premature move to a care home. We are strongly committed to the assessment and rehabilitation of patients who are at high risk of falling.

The Healthcare Assistant role functions as part of the Multi-Disciplinary Team within a community setting to provide a seamless service to patients, with proximal supervision from a registered clinician. The post holder will continue to develop and undertake a range of delegated tasks and will report to a registered nurse or therapist. The Health Care Assistant will provide general / specific care as specified below for a patient group and will be proficient and competent to work across professional disciplines.

  • Deliver clinical and therapeutic care to patients as per care plan
  • Undertake joint clinics with the HARI Consultant
  • Undertake clinical observations for patients attending group exercise classes
  • Support with group exercise classes as required; working closely with other support workers in the team.
  • Assist and support patients in the activities of daily living
  • Use risk assessment tools appropriately to identify and reduce risks to patients and staff to ensure safe practice e. moving and handling
  • Undertake and perform clinical sills and observations against identified competencies to enhance the delivery of patient care e.g. Vital signs, urine testing, blood glucose monitoring, venepuncture, 12 lead ECG recording and deliver personal care
  • Ensure clinical area is prepared and a suitable environment to carry out clinical procedures in the community setting
