An exciting opportunity is available for the right candidate to join LCHS NHS Trust as a Healthcare Assistant Practitioner Apprentice. The right candidate will need to demonstrate that they are highly motivated, problem solvers, demonstrate good time management, and a willingness to succeed using the LCHS NHS Trust values and behaviours. The apprenticeship is a mixture of academic study delivered by our chosen training provider 'Impact Futures' where you will gain new knowledge and skills so that you can apply those skills into your working role.As a Healthcare Assistant Practitioner Apprentice, you will be based at either our Louth or Lincoln sites.
This is a developmental role. The apprentice will work towards progression into the post of Healthcare Assistant Practitioner through successful completion of the Healthcare Practitioner Apprenticeship programme, which includes the attainment of an accredited level 5 occupational competence qualification such as a Foundation Degree, other Level 5 Diploma of Higher Education, or the Level 5 Diploma for Assistant Practitioners in Healthcare. For those Assistant Practitioners who wish to progress into Registered Practice, the apprenticeship can provide credits into some Higher Education programmes aligned to professional registration.
The apprentice will develop and maintain knowledge, skills and competencies related to the role of Healthcare Assistant Practitioner within the service and through completion of the foundation degree apprenticeship programme which may include working in a variety of clinical placements and delivery of person-centred care.
The Healthcare Assistant Practitioner apprenticeship programme combines and integrates both academic and work-based learning through close collaboration between employers and the training provider.
The apprentice will be required to complete an agreed contract approved by both LCHS and the host training provider.

The Apprentice will participate as a member of the multidisciplinary team in the assessment, planning, delivery and evaluation of patients/client’s health and well-being needs under supervision.
Work towards accepting or delegating assessment responsibility, developing individual plans of care to meet the immediate and ongoing needs of the patients and carers.
Work towards the prioritisation and effectively management of clinical time and other responsibilities.
Work towards planning, organising, and prioritising the workload of junior members of staff.
To gain knowledge of specific physical, emotional, psychological, social, cultural, and spiritual needs of the patients and carers and, taking these into account in assisting the implementation of appropriate actions to meet those needs.
Observe and where appropriate participate in the delivery of complex care as prescribed by the qualified practitioner.
Work towards planning and progress patient interventions with autonomy. Assessing and planning daily with indirect supervision of qualified practitioner.
Gain understanding on a range of non-routine work procedures and practises which require intermediate level of theoretical knowledge and physical skills gained through relevant training and experience.
Support in the education to patients/carers to facilitate discharge planning or to promote self-care under supervision where appropriate.
Work towards demonstrating working knowledge of other provider’s resources and referral systems to ensure patient needs are met, within scope of practice.
Work towards developing and consolidating personal knowledge and skills demonstrating the evidence as directed by the education provider.
Working towards demonstrating continuous evaluation of personal clinical practice and identifying opportunities to meet own learning needs.
Adhere and be responsible for the safe use of all equipment within their scope of practice.
To identify needs and advice on equipment and resources, requisitioning where there is delegated authority and ensuring effective on-going use of equipment is monitored.
