TRAVEL REQUIREMENT: (approximately +/- 90%). Reliable transportation is required. Must possess valid USVI driver’s license, and personal vehicle.

Description Summary:

In this fast-paced environment, the Homeless Health Case Manager and Street Outreach Worker will work closely with homeless clients who experience multiple psychosocial and/or health related needs. The HH Case Manager & Street Outreach works on the planning, implementation, coordination, monitoring and evaluation of services. Maintains records, completes necessary forms and collects and analyzes data, as required. Prepares a variety of special and recurring analyses and reports. Serves as a liaison with other organizations and provides information regarding various department services and programs. Supports clients with following through on treatment plans by being flexible in appointment scheduling, arranging for rides to the office and holding clients accountable for their treatment. Designs community education programs and deliver workshops and class instruction to people in the area to help inform the public about the facts of homelessness.

Essential Functions:

  • Responsible for coordinating clinical care for a caseload of 25-30 clients.
  • Become familiar with guidelines on Evidence Based Interventions to serve those who are homeless, at risk of homelessness or in temporary shelters.
  • Develop a schedule to deliver interventions to each client in caseload.
  • Provide ongoing documentation of interventions and outcomes in EHR.
  • Support clinical and behavioral providers in the coordination of services for and supports specific to those who are homeless or housing vulnerable.
  • Engage clients around housing status, health care issues/needs, alcohol and drug use/abuse, HIV status and risk behaviors.
  • Assist Program Coordinator, Providers in establishing community linkages, Saturday clinics and designated venues to expand client base.
  • Assist client in access the services and resources that might be available to them including but not limited to MAP, SNAP, aid to the disabled, etc.
  • 10. Complete all necessary paper work required by HRSA and/or any other grantor requirement and enter data into EvaluationWeb, NextGen and/or other electronic data systems.
  • 11. Be involved in program evaluation activities, and engaged as part of a team committed to continuous improvement of our services.
  • Actively collaborate with other FHC departments/units to develop mutually supportive program wide services, supports, and opportunities for clients.
  • 13. Participate in agency-wide outreach events.
  • On rare occasions, serve as chaperone if off-island medical travel is required.

Position requirements / Job qualifications

  • Must be able to work a very flexible schedule including weekends.
  • Prior case management experience strongly desired.
  • Demonstrated comfort and self-motivated interaction with diverse client base.
  • Experience working with diverse populations.
  • Experience working with an at-risk, homeless and or marginally housed population, highly preferred.
  • Possess valid Virgin Islands Driver’s license and insurable driving record.
  • 7. Strong work ethic and reliability.
  • Team oriented and willing to cross-train and pitch in as needed.
  • Good oral and written communication skills.
  • Experience delivering educational programs preferred.
  • Cultural competence and strong customer service skills essential.

Evaluation:

The evaluation of work performance is ongoing and will be performed by the Director of Social & Community Programs. It will include the specific duties and responsibilities of this position description plus employee’s disposition and general working behavior. Formal evaluations will normally be performed on an annual-basis.

Education and/or Experience:

Minimum requirement - High school diploma. College degree preferred (Behavioral sciences such as Social Work, Psychology; or, Nursing degree or LPN, CMA certification preferred); Minimum 1 year experience in a health care or outreach setting highly preferred.

Language Skills:

Ability to communicate concisely and clearly orally and in writing with clinical staff, patients, and center staff. Bilingual (English/Spanish), highly preferred; American Sign Language preferred. Ability to follow simple, oral, and written directions. Excellent verbal and written communication skills, and ability to lead meetings and conference calls.

Mathematical Skills:

Ability to prepare, read, and interpret documents such data reports, SPSS, Stata, and/or R for data management analysis, and procedure manuals. Ability to calculate figures and amounts.

Reasoning Ability:

Good analytical skills. Ability to solve practical problems and deal with a variety of concrete variables in situations. Proofread documents. Ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form. Ability to prepare, read, and interpret documents such data reports, SPSS, Stata, and/or R for data management analysis, and procedure manuals. Ability to demonstrate good judgment in selecting methods and techniques for obtaining solutions.

Competency/Skills Requirements:

Competency required in the areas of communication, excellent verbal and written skills. Ability to prepare accurate reports. Ability to prepare complex work rapidly and accurately. Ability to type at least 50 words per minute. Computer literate to include Microsoft word, excel and access. Working knowledge (Expertise preferred) in statistical analysis, data visualization, database software, and languages, and Microsoft Access, preferable.

Other Skills and Abilities:

Using Excel to develop visualizations, create pivot tables, and conduct analyses. In conjunction with IT personnel, uses SPSS, Stata, and/or R for data management and analysis. Working with and aggregating/merging multiple data streams. Working with HIV health care or HIV social services delivery. Working with community advisory groups. Working with people from diverse backgrounds. Expertise in statistical analysis, data visualization, database software, and languages, including .SQL, and Microsoft Access, preferable. Excellent verbal and written communication skills, and ability to lead meetings and conference calls. Strong organizational and interpersonal skills with the ability to work both independently and as a team member. Ability to demonstrate good judgment in selecting methods and techniques for obtaining solutions.
