Are you a Physical Therapist(PT) looking for extra income while working on a flexible schedule?

Here at Creek View Home Health & Hospice, we are looking to immediately hire PTs. This is a per diem position paid on a "per visit" basis.

.Your visit will involve providing skilled patient care at the patient's home. It is a low pressure environment that allows you to focus fully on the care of the patient.

As of this moment we are looking for someone who can ideally cover all or most of the of the following areas in Phoenix: Central Phoenix, Mesa, Gilbert, Tempe, Laveen, Avondale.

Position Requirements:

• Active PT license in the state of Arizona.
• A least 1 year of home health experience.
• A valid and up to date CPR certification.
• A reliable mode of transportation. This position requires you to drive to the patient’s home.

Job Types: Part-time, Per diem

Pay: $75.00 - $100.00 per hour
