The Registered Nurse Case Manager is responsible for providing evaluation and nursing services on an intermittent basis in the patient's place of residence. Functions independently and/or in conjunction with an interdisciplinary team based on the patient's identified needs. Practice is within the scope defined by the Washington State Nurse Practice Act, based on the nursing process model and concept of community health nursing.

Required qualifications:

• Graduate of an accredited school of nursing
• Required upon hire: Washington Registered Nurse License
• Required upon hire: National Provider BLS - American Heart Association
• 1 year full time experience practicing as a Registered Nurse

Preferred qualifications:

• Bachelor's Degree
• 1 year recent (within last three years) clinical experience in a Medicare certified Homecare or Hospice agency/facility doing intermittent visits and case management
