SOCIAL SERVICE DESIGNEE - PSYCHIATRY - BEHAVIORAL HEALTH

Social Service Designee (SSD) - Behavioral Health - 8 hour shift

Seeking a Social Service Designee to work on the 12-bed inpatient Geriatric Psychiatry Unit in the South Central Kansas Regional Medical Center in Arkansas City, KS. Arkansas City is one hour from Wichita, KS and two hours from Tulsa, OK.

Job Duties Include:

• Maintaining supportive relationships with patients and families
• Monitors on a daily basis the status of patients and communicates status to team members
• Coordinating with interdisciplinary team to develop individualized treatment plans
• Accurately documenting pertinent behavioral and psychosocial information in progress notes
• Maintains regular, timely and appropriate documentation in patient charts regarding services provided including actual and attempted contacts with patient, family, and physician
• Works closely with treatment team to ensure continued stay is justified
• Documents on the patient chart the specifics of problems associated with implementing the discharge plan and actions being taken to solve problem(s) including the overall effectiveness of the action plan.
• Assesses and reassesses the needs of the patient and accurately documents assessments and discharge needs
• Notifies psychiatrist for discharge order, aftercare appointments and prescriptions
• Assesses the need for patient and/or family referral to appropriate resources after discharge as a means of providing continuum of care and maintaining or improving function and at the request of the treatment team
• Completes and coordinates discharge with the treatment team and physician within the designated time frame
• Completes discharge planning forms and patient discharge forms by date of discharge
• Review patient advanced directives on a routine basis and provide education and resources to patients regarding advance directives
• Reviews and discusses discharge plan with patient and/or family receiving input from them as well as providing explanation, information and support for acceptance and implementation of the discharge plan and obtains signature for involvement in treatment planning
• Completes and coordinates discharge with the treatment team and physician within the designated time frame
• Review patient advanced directives on a routine basis and provide education and resources to patients regarding advance directives

Requirements of Social Service Designee:
Current social service designee certification or licensed social worker.
Preferred minimum one-year experience in working behavioral health services.
Excellent interpersonal skills, written and verbal communication skills and Basic computer skills.
