Job description

The Stroke Data Collector will, in conjunction with colleagues in the Stroke Department, provide a comprehensive information and data collation service relating to all identified stroke patients. This includes patients admitted to and discharged from the stroke unit and within the Trust along with all patients attending the TIA Clinic as outpatients/inpatients.

The key purpose of this role is to ensure the accuracy of the data collected on all steps of the patient pathway including SSNAP, Stroke Register, TIA Register and other stroke medicine related databases/spreadsheets. As such, the post holder will be required to validate information held on the Trusts various information systems and assist in collecting the relevant data which will involve reading patients care pathways, patients notes and Doctors, nurses and Therapists notes.

As part of their role, they will assist in the collection, capture and analysis of various reports and audits and provide data for other departments and national standards including KPI’s and any other stroke related data requirements.

The post holder will work as part of a team and will assist and support the Stroke Data Coordinator as requested.
