Position Description: The position will function as a member of a team to implement Tiered Care Coordination and High Fidelity Wraparound Services (TCC/HFW) for Robeson and Scotland children and youth with involvement in the Juvenile Justice System and Department of Social Services who have mental health diagnoses. This position will rotate locations at Scotland County DSS, Robeson County DSS and Juvenile Justice. This person will address critical community needs of individuals and works one-on-one with the youth or family member/caregiver(s) in order to provide information and support throughout the care planning process participating in Tiered Care Coordination. This person will also foster relationships and partnerships to assist in sustaining a System of Care approach for youth and families on how to effectively navigate the systems and facilitate the youth and parent /caregiver’s access to information/community resources. Family Navigators are staff members who are also the parent or primary caregiver of a child who has/had mental health or substance abuse challenges. As such this position does require that candidates have lived experience.

Responsibilities:

• Participate as a member of the Tiered Care Coordination team.
• Build partnerships with public and private mental health agencies, child serving agencies, DSS, DJJ and community partners to value and invest in family involvement activities in an effort to sustain family involvement through the establishment of natural supports.
• Assist families in linking and advocating for appropriate services, in and out of traditional systems.
• Work in partnership with Systems Partners such as DSS, DJJ and family organizations to link families to appropriate services and supports.
• Provide ongoing technical assistance to build family leadership, family and professional partnerships, and implement family driven practices.
• Participate in the ongoing strategic planning and work plan implementation for the Tiered Care Coordination Program.
• Work in partnership with the project team to implement the day–to–day activities of the grant and participate in all administrative team meetings and discussions.
• Provide outreach to family members of children not receiving services served through the MCO and referred by DJJ/DSS.
• Participate in all-mandated meetings for Tiered Care Coordination.
• Represent MCO in beneficial community meetings to support the System of Care philosophy.

Education/Experience/Licensure: High school diploma or GED and a minimum of two years of experience working with children/adolescents/transition age youth. Experience in navigating any of the child and family-serving systems and teaching family members who are involved with the child and family serving systems. Must have lived experience as a primary caregiver for a child who has/had mental health or substance abuse challenges. National Certification in Family Peer Support or willing to actively work on completing certification and is on track to complete Family Peer Support certification within one year of hire date.
